Youth in Transition to Adulthood
First Nations Health Consortium values the ideals, imagination, and energy of young people as they are vital to the continuing development and healing of First Nations communities and society at large.
Our Youth in Transition to Adulthood services are about making connections and linkages between young people and required support services.
- Provide referrals and/or navigational services to community agencies for support and guidance
- Assistance with applications, referrals, and information provided re: Government agencies
- Preventative community needs such as training, presentations, and workshops
- Identify gaps in services for young people who are aging out Jordan’s Principle and other services in the community, regardless of residency (on or off-Reserve)
- Participate and facilitate in transition meetings with families, service providers and other professionals
- Working collaboratively with Regional Service Coordinators to assess and determine supports available to families and youth
- Youth Transition Coordinators assess, plan and collaborate to provide options and services required to meet the needs of youth in transition
Youth Transition Coordinator will work with young people: Ages 14-thru early adulthood to identify services or potential gaps in services that are essential to transitioning to adulthood.

- Child and family services involvement
- Band Designate inter-agency meetings
- Support and guidance for Semi-Independent Living (SIL) options
- Housing Program opportunities
- GOA program applications needed (AISH, PDD, OPTG, Income support)
- Bridging the family to necessary agencies
